How to establish relationships between Objects and the Simple Survey Object

In order to create survey programs, the Simple Survey Object needs to have a relationship with the Object that you will be surveying from.

Details
A relationship can be established directly from the Survey Builder while you're configuring a new survey program. 

If when you select the Survey From Object, a message saying 'No relationship exists from Survey to ObjectName', simply click on the Create Relationship button to establish one.
