To open up access to the fields and related SObjects, please follow these instructions:

Navigate to Salesforce Setup and find the Simple Survey Site Guest User 

Click on the Simple Survey Profile to edit it:

Select Object Settings in the Apps section:

Select the SObject the merge field is part of. In this example, we will use Accounts.

Select the 'Read' checkbox in the Object Permissions section:

 

Click 'Save'.

The merge fields should now be correctly populated if values are found.

NOTE:  The 'Read' permission is not granted to the Simple Survey Site Guest User user, the values for the merge fields cannot be read and will not be populated. The 'default text' entered during the merge field creation in the Simple Survey Builder will be used instead.
