1. Go to Setup > User Interface > Sites and Domains > Sites
2. Click on "Simple Survey" under the heading Site Label
3. Click the Edit button
4. Uncheck Active
5. Change Active Site Home Page from the Visualforce page "Simple Survey" to "InMaintenance"
6. Click Save

If you have installed the Simple Survey extension package, which contains workflow rules to automate survey delivery, you will also see a third error related to this package. Simply uninstall the extension package before attempting to uninstall Simple Survey.

If during the uninstall process you receive errors related to Reports, but cannot find them, they are probably private reports created by other users. You can run the following query in the developer console to identify who is the owner, and have them manually delete the reports: SELECT Id, Name, CreatedById, LastModifiedById FROM Report USING SCOPE allPrivate

If uninstallation fails due to images, please delete the Simple Survey documents folder in Salesforce Classic.

Simple Survey will now uninstall.  If you continue to have issues with uninstalling Simple Survey, please contact simplesurvey@vicasso.com.
