Simple Survey configurations for Setup Wizard and Survey builder not reflecting correctly

You notice your surveys are not reflecting the configurations as they were set up in the Setup Wizard and Survey Builder.

Symptoms
1) The landing page is not reflecting your custom branding settings, displaying something similar to the following:

2) The Survey Record Owner defined in the Survey Builder is different than the one on the Survey Record page.

3) The Survey Record Type is set to 'Default' on the Survey Record page, not reflecting the actual survey name.

4) Prevent Duplicates is not working as expected. 

5) Unable to answer questions on the survey landing page.
Solutions
This usually happens if the Simple Survey Site Guest User does not have its needed permissions. You will need to make sure that the Site Guest User has a Simple Survey License and the Simple Survey Site Guest User Permission Set assigned to it.

In the Simple Survey Setup Wizard, navigate to the Initial Setup tab and click on the "Add License and Permission Set" button located in Step 3 of the Configure Force.com Site section:

If that doesn't work, please open a case with Vicasso Support to report this issue. Alternatively, go to Salesforce Setup | Look for the Simple Survey Site Guest User | Confirm that it has a Simple Survey license and the Simple Survey Site Guest User permission set assigned:
